Free vs paid at a glance

Free: walking the nave and side aisles to view the Pietà and Baldacchino (queue via St Peter's Square security, like airport security).

Paid/official booking: Dome climb, Treasury Museum, audio guide, and guided entries — all through the Fabbrica di San Pietro.

Third-party sites selling a "Basilica entry ticket" alone are unreliable — there is no official standalone nave ticket.
